         This was a quasi-experimental study. Results showed OOP payments in the hospitals affiliated to the Ministry of Health and Medical Education (MoHME), Social Security, and private sector, respectively, were 24%, 15.8%, and 30.5% before the implementation of the HTP. OOP changed to 5.8%, 10.8%, and 35.8%, after the implementation of the first phase, and 3.4%, 3.6%, and 6.5% after the implementation of the third phase of the HTP. The prevalence of informal payments to physicians in hospitals affiliated to the MoHME, Social Security Organization, and private sector, was 4.5%, 8.1%, and 12.5% respectively, before the implementation of the HTP, and 0%, 7.1%, and 10% after the implementation of the first phase of the HTP. No informal payment to physicians was reported after the implementation of the third phase of the HTP. Considering the results of this study, it seems that the implementation of the HTP has significantly reduced the OOP payments in Iran. HTP has also contributed to the reduction of informal payments to physicians, at least in the short run.
